History of Present Illness

Patient reports she had a tooth extracted a few years ago, and she had her implant placed 4 months ago and wanted to finish treatment. Upon exposing the implant, the implant failed to integrate and was explanted and the site was bone grafted and allowed to heal for 5 months before a second attempt of implant placement.
